25-26 Winter League Waiver

ASSUMPTION OF RISK, WAIVER, AND RELEASE OF LIABILITY

In consideration of being allowed to participate in the Shoot Hoops not Bullets basketball league (“the League”) organized by Eagles Nest YA and Fairfax NCS (the “League Organizers”), I, the undersigned, agree to the following:

1. Voluntary Participation and Assumption of Risk

I acknowledge that participation in basketball and related activities involves inherent risks, including but not limited to the risk of serious injury (e.g., sprains, fractures, concussions), illness, or even death. I freely assume all such risks, both known and unknown, even if arising from the negligence of others, and take full responsibility for my participation.

1. Release and Waiver of Liability

I, for myself, my heirs, assigns, personal representatives, and next of kin, hereby release and hold harmless the League Organizers, league staff, referees, facility owners, sponsors, volunteers, and other participants from any and all liability for injury, disability, death, or loss or damage to person or property incurred while participating in the League.

1. Medical Treatment

I authorize the League Organizers or their designees to provide or seek medical treatment on my behalf if I am injured or otherwise incapacitated. I understand that I am responsible for any medical costs incurred.

1. Insurance

I acknowledge that it is my responsibility to obtain adequate health insurance coverage. I understand that the League does not provide medical or accident insurance.

1. Code of Conduct

I agree to abide by all rules, regulations, and codes of conduct established by the League. I understand that failure to do so may result in disciplinary action, including suspension or expulsion from the League.

1. Media Release

I grant permission to the League Organizers to use photographs, video, or audio recordings of me taken during League activities for promotional, social media, or educational purposes without compensation.
