PHOTO CONSENT AND MEDIA RELEASE FORM
ORGANIZATION: SoCal Heat Softball

PHOTO AND VIDEO CONSENT
I hereby grant SoCal Heat Softball, its employees, agents, volunteers, and authorized representatives the irrevocable right and permission to photograph, film, videotape, or otherwise record the participant named above during softball activities, events, games, practices, and related functions.

USE OF IMAGES AND RECORDINGS
I understand and agree that SoCal Heat Softball may use such photographs, films, videotapes, recordings, or other likenesses for the following purposes:
- Promotional materials and advertisements
- Website content and social media posts
- Newsletters and publications
- Training and educational materials
- Fundraising activities
- News media coverage
- Any other purpose deemed appropriate by SoCal Heat Softball

RIGHTS AND RELEASES
I waive any right to inspect or approve the finished photographs, films, videotapes, recordings, or other materials that may be used in connection with the images, or the use to which they may be applied. I release SoCal Heat Softball from any claims, damages, or liability arising from the use of such images or recordings.

NO COMPENSATION
I understand that no monetary compensation will be provided for the use of these images or recordings.

OWNERSHIP AND COPYRIGHT
I acknowledge that SoCal Heat Softball will own all rights, title, and interest in and to the photographs, films, videotapes, and recordings, including copyright thereof.

REVOCATION
I understand that this consent remains in effect until revoked in writing. To revoke this consent, I must provide written notice to SoCal Heat Softball.

MINOR PARTICIPANTS
If the participant is under 18 years of age, I certify that I am the parent or legal guardian and have the authority to grant this consent on behalf of the minor participant.

ACKNOWLEDGMENT
By signing below, I acknowledge that I have read and understood this Photo Consent and Media Release Form and agree to its terms.
