Participation Policy/Waiver

My child's participation in the program selected is voluntary. I understand that the selected activities may involve accidental injury and hereby voluntarily assume such risks. Knowing these risks, I want my child to participate in this activity. I (on behalf of my child) hereby assume the risk, and hereby waive, release, and discharge Teana Edwards aka L8dyTee, Soles on the Ground-its officers, employees, coaches, activity instructors, assistants, and volunteers. In addition all officers, and employees of the school, park, residence, business location or community center sites where said activity will take place, from any and all claims for damages, for personal injuries, medical bills or claims for damages to property, which my child or my child's heirs, assigns, executors or administrators may have or which may accrue to my child's participation in this activity.

CLASS CONDUCT: Soles on the Ground reserves the right to release, without refund of program fees, any child whose behavior becomes unmanageable.

REFUND POLICY: Refunds will not be issued for cancellations or withdrawals. Under extenuating circumstances, a refund may be issued at the discretion of the head coach.

WEATHER: We are a rain or shine club. If weather does not allow field use, head coach will relocate indoors if available. If snow prohibits safe traveling or participation, class will be cancelled.

MAKE-UP CLASSES & TRANSFER POLICY: Make up classes & transfers will be accommodated as long as there is room in the class.

SICK POLICY: If child, sibling, or anyone in the household is sick please do not attend session and schedule a make up. If child is vomiting, has diarrhea, or has a fever wait 24 hours after symptoms have subsided without medication before attending session (ie: symptoms Wednesday through Saturday, symptom free Sunday can attend class Tuesday) Sick children will be sent home.

COVID-19: I am fully and personally responsible for my childs safety and actions while and during participation and I recognize that they may be at risk of contracting COVID-19. With full knowledge of the risks involved, I hereby release, waive, discharge the Organization, its board, officers, independent contractors, affiliates, employees, representatives, successors, and assigns from any and all liabilities, claims, demands, actions, and causes of action whatsoever, directly or indirectly arising out of or related to any loss, damage, injury, or death, that may be sustained by me related to COVID-19 while participating in any activity while in, on, or around the premises or while using the facilities that may lead to unintentional exposure or harm due to COVID-19. I agree to indemnify, defend, and hold harmless the Organization (including but not limited to Soles on the Ground) from and against any and all costs, expenses, damages, lawsuits, and/or liabilities or claims arising whether directly or indirectly from or related to any and all claims made by or against any of the released party due to injury, loss, or death from or related to COVID-19

LATE POLICY: Child will need to be picked up at the end of the scheduled class. Effective immediately a late pick-up charge of $10.00 for every 10 min
