I waive all responsibility from any members of the Stratford Hoops Club or Stratford School.
I also waive responsibility for any stolen, lost, or broken items from any members of the Stratford Hoops Club or Stratford School District during or resulting from activity in this tournament.
I also permit the officials or supervisors to use their best judgment if any injury does occur and to help or refer the athlete to proper health care providers.
