Consent To Participate In Tdyfc Youth Sports Programs And Activities:

By registering here, I, the undersigned parent(s) or legal guardian(s), hereby give my approval for my child's participation in any and all The Dream Youth Football Conferenceyouth sports programs or activities. I acknowledge that my child will participate in athletic activities that could lead to serious injury. I hereby grant permission for my child to participate in any TDYFC youth sports programs or activities and I have no knowledge of any reason that Participant is not fully physically able to participate in all camp activities.
Photography Release:
By registering here, I hereby consent to the photography of my child by TDYFC, Samuel L. Felton Community Development Program Inc. Department and/or it' agents. I hereby consent to the use of these photographs singularly or in conjunction with other photographs or video recordings for TDYFC and/or Samuel L. Felton Community Development Program Inc. marketing purposes without any compensation.

Understanding of risk:
Furthermore, I, the undersigned parent(s) or legal guardian(s), hereby state that I understand the seriousness of the risks involved in participating in this program, my personal responsibilities for adhering to rules and regulations, and accept them as a participant.

2 - CONSENT FOR MEDICAL TREATMENT, AUTHORIZATION & RELEASE
By registering here, I, the undersigned parent(s) or legal guardian(s), recognize that all activities of a physical nature involve some risk of injury and by registering for an activity of this nature there is an assumption of risk by the participant. While particular rules, equipment and personal discipline may reduce this risk, the risk does exist. Additionally, if my child is injured, I authorize the Columbia Youth Football League, Inc. officials, coaches and representatives to obtain immediate medical care for my child. I, the undersigned parent(s) or legal guardian(s) and child, knowingly and freely assume all such risks associated with this program.
In the rare event of serious accident or illness, it is the policy of The Dream Youth Football Conference(TDYFC) and Samuel L. Felton Community Development Program Inc. to (1) contact emergency dispatch (911) to perform first aid and when necessary recommend transport of the victim to a hospital; (2) contact parent or guardian as soon as possible as the situation allows.
I hereby authorize the physicians/providers and staff of any medical facility to provide the above-named Participant with any and all medical treatments including examinations, x-rays, tests, anesthesia, operations and diagnostic procedures, which may be deemed necessary or advisable by the attending physician and/or surgeon.
Persons desiring a different procedure must notify TDYFC in writing at the time of registration. Parents who do not agree nor sign the waiver must be in attendance at all games and practices.

Medical insurance:
By registering here, I, additionally assume all financial responsibility for any and all medical related costs. I, the undersigned, shall ensure that my player participant is covered under our own medical insurance. All TDYFC youth sports program participants are responsible for their own insurance. TDYFC does not provide any medical insurance for players or program participants.

3 - WAIVER AND RELEASE OF LIABILITY FOR MINOR PARTICIPANTS
IN CONSIDERATION of my child being allowed to participate in any way in the the Dream Youth Football League youth sports football program, related events and activities, the undersigned parent(s) or legal guardian(s), acknowledges, appreciates, and agrees that:

1. The risk of injury to my child from the activities involved in these programs is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,

2. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the TDYFC or others, and assume full responsibility for my child's participation; and,

3. I willingly agree to comply with the program's stated and customary terms and conditions for participation. If I observe any unusual significant concern in my child's readiness for participation and/or in the program itself, I will remove my child from the participation and bring such attention of the nearest official immediately; and,

4. I, for myself on behalf of my/our heirs, assigns, personal representatives and next of kin, hereby release the other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event, hereafter referred to as Releasees, with respect to any and all injury, disability, death, or loss or damage to person or property incidental to my child's involvement or participation in these programs, whether arising from the negligence of the Releasees or otherwise, to the fullest extent permitted by law.

5. I, for myself, my spouse, my child, and on behalf of my/our heirs, assigns, personal representatives and next of kin, hereby agree to assume the risk and hold harmless, release, and otherwise indemnify Columbia Youth Football League, its officers or agents, its affiliated organizations and sponsors and the owners of, in the performance of said fields and facilities utilized for the programs, from any liability from any and all liabilities incidental to my involvement or participation in these programs, even if arising from their negligence, to the fullest extent permitted by law.

I have read this release of liability and assumption of risk agreement, fully understand its terms, understand that I have given up substantial rights by signing it, and sign it freely and voluntarily without any inducement.

6 - GENERAL FINANCIAL POLICY
Registrations and Player Team Assignments:
Only teams/organizations who have paid their registration fees in full will be placed on tournament brackets.
Refund Policy: The policy of the TDYFC is to issue NO REFUNDS after a team has played in a sanctioned game or practice. Prior to playing in a sanctioned game or practice, a refund of the registration fee less an administrative handling fee of $25 will be granted.
Returned Checks: All returned checks will be submitted to the Office of the Lorain County Prosecuting Attorney for collection. We may collect a "reasonable" $25.00 service charge, in addition to the face amount of the check, if your check is returned. No exceptions.
