WE hereby consent to be registered with the Lusaka Weekend Championship League as the above named club for the season entered. WE declare that we are not currently registered for any other league, or have any outstanding financial liabilities to any other club in this League. WE agree to be bound by the Rules of the League and understand that ANY breach of these rules will be subject to penalties as defined by the Management Committee of the League.
