The Twelve Baseball Tryout Waiver and Release of Liability
We are looking forward to seeing your son at The Twelve Baseball Tryouts! Please make sure he wears a jersey with his last name clearly visible on the back. Tryouts will begin promptly at 10:00 AM, so please plan to arrive early to allow time for check-in and warm-up.
This Waiver and Release of Liability is executed by the undersigned parent or legal guardian in connection with their child’s participation in tryouts for The Twelve Baseball Program. Participation in tryouts is voluntary and involves inherent risks. This agreement is intended to acknowledge those risks and release The Twelve Baseball and its affiliates from liability.
I understand that participation in baseball activities involves physical exertion and the risk of injury, including but not limited to running, throwing, sliding, catching, and potential collisions. I voluntarily assume all risks, known and unknown, related to my child’s participation in The Twelve Baseball tryouts.
I hereby release and hold harmless The Twelve Baseball Program, its coaches, staff, volunteers, agents, representatives, and affiliated organizations from any and all claims, liabilities, damages, or expenses arising out of or related to any injury, loss, or harm—up to and including death—that may occur during tryout activities or travel to/from the event.
In case of emergency, I authorize The Twelve Baseball staff to seek appropriate medical care for my child. I understand and accept full financial responsibility for any and all medical services rendered. I affirm that my child is physically capable and has no medical conditions that would prevent them from participating safely.
I understand that The Twelve is a Christ-centered program committed to teaching through faith and character. All participants, coaches, and families are expected to demonstrate respect, integrity, and encouragement. Any form of unsportsmanlike behavior or language may result in immediate removal from the tryout process.

By signing below, I acknowledge that I have read, understood, and agree to the terms of this waiver and release. I affirm that I am the legal parent or guardian of the participant and give my consent for their participation in The Twelve Baseball tryouts.
