TOWNBEEF BASKETBALL LEAGUE PLAYER WAIVER AND RELEASE OF LIABILITY FORM

1. Acknowledgment and Assumption of Risk
   I, the undersigned, acknowledge that participation in the TOWNBEEF Basketball League ("the League") involves inherent risks, including but not limited to physical contact, strenuous activity, and potential for serious personal injury (including permanent disability or death), property damage, and other risks associated with competitive athletic activities. I voluntarily assume full responsibility for all such risks.

2. Waiver and Release
   In consideration of being permitted to participate in the League, I hereby release, waive, and discharge TOWNBEEF Sports Organization, its directors, officers, employees, agents, volunteers, sponsors, affiliates, event officials, and other participants (collectively, “the Released Parties”) from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, disability, illness (including COVID-19 and other communicable diseases), loss, or damage that I may sustain while participating in League activities, whether caused by the negligence of the Released Parties or otherwise.

3. Medical Treatment
   I understand that the League does not provide medical or accident insurance and that I am solely responsible for my own medical coverage. I give consent for emergency medical treatment in the event of injury or illness, and I agree to be financially responsible for any such treatment.

4. Code of Conduct
   I agree to conduct myself in a sportsmanlike and respectful manner at all times. I understand that violent behavior, unsportsmanlike conduct, or violations of League rules may result in suspension or permanent removal from the League without refund.

5. Media Release
   I grant TOWNBEEF Basketball League permission to use my likeness, name, image, voice, and/or appearance in photographs, video recordings, social media posts, promotional materials, and broadcasts for any lawful purposes without compensation.

6. COVID-19 and Public Health Acknowledgment
   I understand and acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by attending or participating in League events. I agree to follow all League and venue health and safety protocols.

7. Governing Law
   This Waiver shall be governed by and interpreted in accordance with the laws of the state in which the League operates. Any legal action arising from participation shall be brought in the courts of that state.

I HAVE READ AND FULLY UNDERSTAND THIS WAIVER AND RELEASE. I UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT AND SIGN IT VOLUNTARILY.
