Lewis County Homeschool Athletic Activities (LCHAA)

Supervision of this program is under the direction of the volunteers and parents of Lewis County Homeschool Athletic Activities (LCHAA) group.

All participants are responsible for their conduct to LCHAA Sports Volunteers, Parents and to other persons supervising this program.

I, the undersigned parent or guardian of the participant named below recognize the dangers present in youth sports, playing soccer, and all associated activities.

I believe the LCHAA Volunteers, Parents and their assistants are dependable and reliable and will provide safe experiences.

I understand that during the course of playing youth sports like soccer, my child may be asked to perform drills, practice game scenarios and learn game dynamics.

I hereby grant permission for my child to participate in the LCHAA Soccer program, or event. I knowingly and freely assume all such risks, for example: bodily injury as well as loss or damage to property.

I understand as the parent/guardian signing this form that I will be held financially responsible for any expenses incurred during the course of play.

I assume all risks involved while using equipment supplied in this program or with equipment brought by my child or other children. Participants are responsible for their own equipment.

This is to certify as parent/guardian of this participant, I do consent to his/her release of the Homeschool Youth sports volunteers, other participants and volunteers and the other organizations providing and/or sponsoring the meeting facilities and/or organizations’ volunteers and equipment from any and all liabilities to his/her involvement in the Homeschool youth Sports Program.
